Alpha Omega Dental Center would like to thank you for choosing us as your healthcare provider.  We are committed to you and your treatment.  Please understand that payment of your bill is considered a part of your treatment.  The following is a statement of our Financial Policy, which we require that you read and sign prior to any treatment.

Out of Network Providers
Alpha Omega Dental Center is an out of network provider for all medical insurance policies, therefore payment will be due at the time of service.  We have opted out of submission into Medicare as well.  We will send all claims to your insurance company, excluding Medicare, and will assist you in getting reimbursement.  Many insurance plans require a referral from your physician in order to get coverage, so you may need to contact your medical physician’s office to have the referral and prescription for an oral appliance sent to our office prior to any treatment.

We strongly recommend that you contact your insurance company prior to treatment to confirm the amount or percent of coverage to expect from your medical insurance company for the medical care received in this office.  This information will let you know what your net responsibility will be prior to beginning treatment.
